For those who are Medicaid eligible, but who have been overlooked or who have been otherwise denied coverage for community based long term care, IOCC has developed a home care program called Medicaid Safe Harbor. It is a formal agreement between IOCC and Medicaid that allows people with Medicaid to receive home care that is paid for by the Medicaid program in areas where IOCC provides these services regionally. In order to qualify for these services, people must be certified Medicaid eligible by either their Medicaid or their private insurance plan. From the time the home care application receives approval through initiation and completion of the program, usually over 60 days, people receive the highest quality services for the lowest possible cost.
Within the first 30 days of the Medicaid Safe Harbor program being available, the IOCC team was able to provide home care to over 4,300 people in 53 jurisdictions. This includes people who are in senior residence homes, who are employed, and who are Medicaid eligible.
The services come in two forms: service visits and services paid for by Medicaid. The visits may be caregiver visits to enable the beneficiary to remain out of a hospital or long term care facility or, just need someone to visit once a month to see that everything is ok, or they may be caregiver visits to get the beneficiary’s independent living skills back into shape. The services paid for by Medicaid are: skilled nursing for people who need help with daily activities, physical therapy, occupational therapy, and speech and language therapy. Home health aide services provide social and personal services to help the beneficiary maintain their independence and to provide caregivers with supervision and support.
IOCC has established five regional entry points in North America where people can apply for home care. These are Albany, NY; Houston, TX; Jacksonville, FL; Seattle, WA; and Memphis, TN. Each entry point is led by a trained community team leader. All home care decisions are made by the community team leader.
